Transitioning To A New Home Made Easy

We know that transitioning into a new accomodation can be daunting because there are a lot of new changes.

For this reason, we made sure that our Disability Accomodation Intake Process involves our Participants as much as possible.

This is integral for the Participants as it introduces as much choice, familiarity and comfort as possible in the process for the Participant.

Our clients are the centre of every step towards finding them accomodation they love.

1. Initial Meet And Greet For The Disability Accomodation Intake Process

We know that transitioning into a new accomodation can be daunting because there are a lot of new changes.

For this reason, we made sure that our Disability Accomodation Intake Process involves our Participants as much as possible.

This is integral for the Participants as it introduces as much choice, familiarity and comfort as possible in the process for the Participant.

2. Matching

We then have a team meeting and use this information to create a profile of our new client and their ideal living arrangement; room mates, carers, and the home that most suits them.

3. 1:1 Starting Transition Supports During The Disability Accommodation Intake Process

There are a lot of new changes happening, so before the client moves into their new home, we arrange for support to start a few weeks prior; 

This ensures that our participants have developed a good relationship with their carers and it makes the transition process much easier because there is one less change that that they need to deal with.

4. Discuss The Arrangements

Our client is happy, so it’s time to discuss the final arrangements for the Disability Accomodation Intake Process. This includes things like the roster of care, moving date, is there any homely touches that the client wants (furniture etc), financials, who the carers will be etc.

5. Client Moves Into Their New Home

And it’s that simple.
